Student Registration for Grades 1 – 7

Please come to the office to fill out a registration form and a parent declaration form. Office is open from 9:00am -11:30am and 1:00pm – 3:00pm.You will also need to bring the following documents (photocopies will NOT be accepted):

For verification that Jarvis is your catchment area school, please check the Catchment Area Map or the School Locator.

Registration Documents Required for all Students:

• Appropriate Proof of Citizenship or status in Canada for both students and parents, and appropriate proof of age and guardianship of the students:
  –  Canadian Birth Certificate (showing the parents’ name(s))
  –  Canadian Passport
  –  Permanent Resident Card, immigration or refugee documentation
• Appropriate Proof of Residency:
  –  Purchase Agreement, Current Year Property Tax Assessment
• –  Rental/Lease/Tenancy Agreement, Verification by Landlord,  Current Month Rental Receipt showing address, or Letter from Landlord, along with a copy of one of the following showing rental address:
  –  Utility Bill
  –  Income Tax Assessment
  –  Government Issued Cheque
  –  Vehicle Ownership Papers
  –  Insurance Certificate
• Most recent report card from previous school, if available.
